When booking your appointment, please be prepared to supply the name of your vision insurance company, the name of your plan, and your ID number. This information will help us in case we need to obtain pre-authorization from your insurance company before performing your exam.
At the time of your appointment, we will ask you to show your vision insurance ID card or provide other proof of vision insurance coverage. Depending on your vision plan, we may also ask you to pay a deductible, co-pay, and/or discounted prices for the services you receive.
